By signing this form, I acknowledge and agree to the following:
Communication Consent
I consent to receive email updates, newsletters, event invitations, and research-related information from End AxD.
I understand I can opt out at any time by clicking "unsubscribe" in emails or contacting info@endaxd.org.
Data Privacy & Usage
My personal information will be stored securely and used solely for purposes related to End AxD's mission, including advocacy, fundraising, and research updates.
My data will not be sold, shared, or used for third-party marketing purposes.
I acknowledge that aggregated, anonymized data may be used for research and awareness efforts.
Photo & Testimonial Release (Optional)
☐ I grant End AxD permission to use my name, photos, and/or testimonials in promotional materials, including social media and website features.
☐ I do not grant permission for my photos or testimonials to be shared.
Medical & Research Engagement (If Applicable)
I understand that if I provide medical or research-related data, it will be handled in accordance with End AxD’s privacy policy.
I acknowledge that participation in any research or surveys is voluntary, and I can withdraw at any time.
Minors & Legal Guardians
If signing on behalf of a minor or a legally incapacitated individual, I confirm that I am authorized to provide consent.
